A McKees Rocks man who witnessed the five-car crash Wednesday afternoon near the Fort Duquesne Bridge said one of the drivers killed was speeding and narrowly avoided another major collision just seconds earlier.

“We were behind him doing 60 (mph) and this guy just left us in the dust,” Nicholas Hunter, 46, told TribLive on Thursday.

Video taken by Hunter and shared with TribLive appears to show the black SUV driven by Michael Smith, 52, of Sheraden traveling at high speed along Route 65. Hunter was in the back seat of a car driving behind him. He began recording because, in his retelling, the SUV clipped the median heading onto the highway.

“I thought he was going to cause an accident,” he said.

Hunter pointed the phone down just as the SUV veered right toward the bridge, missing the moment when it careened off a ramp and plunged about 30 feet onto Reedsdale Street below.

When the roadway came back into view of the video, the SUV was nowhere to be seen, and four cars were damaged with debris scattered all over the road.

Authorities say Smith was ejected from his vehicle around 3 p.m. and died instantly.

Danielle Jackman, 58, of Churchill, whose car remained on the ramp, was also killed.

Cara Cruz, a Pittsburgh police spokeswoman, said Hunter’s videos are part of the investigation.

Hunter said he ran up to Jackman’s vehicle right after the crash, which mangled the driver’s side door and crunched the passenger’s side against a barrier, making it impossible to get her out by hand.

Ambulance crews had to use the Jaws of Life to free her.

Jackman was unresponsive, as seen in additional footage from Hunter. Another woman stood by his side, reaching into a gap in the twisted metal to hold Jackman’s hand until medics arrived.

Jackman was taken to UPMC Mercy hospital in Pittsburgh’s Uptown neighborhood, where she was declared dead within an hour of the crash, the Allegheny County Medical Examiner’s office said.

About 10 people were involved in the crash but not injured.

Pittsburgh police are handling the investigation, despite the incident occurring on a state road.

Determining the exact cause of the crash could take a couple of weeks, but speed may have been a factor, Cruz said.

“We all know it takes a pretty significant amount of speed and force to flip over a bridge deck,” Pittsburgh police Chief Jason Lando said Wednesday.
